My Mazda Protege5's brakes are making a screeching sounnd and the rotor is

scored with a circular groove... A little history: I got new brake pads from Sears in June, and the screeching (sounds like rusty bed springs as soon as I hit the brakes) noise started and then a groove formed a week or two later. Sears said rotor was warped and turned it (It's the front passenger one), but the noise persisted. They couldn't fix it so I took it to the Mazda dealership. They ended up putting in new Mazda pads, and replaced the rotor and calipers. (That's about everything, right?) The noise stopped, but then it came back a week later. I brought it back in, but the brakes were quiet when I showed up (Argh!). They did a brake inspection anyways and said everything was fine. The noise started again, and now THIS rotor has that groove in it, just like the one that they replaced. Any ideas? I've already spent way too much money on this. PLEASE HELP.
